## Key Facts

- **Credit check:** Applying performs a hard credit inquiry — this is standard for any credit card application and may have a temporary effect on your score. There is no guaranteed approval.
- **Interest-free period:** Approved accounts are eligible for a promotional interest-free period of up to 15 months. After the promotional period, a standard rate applies based on the applicant's individual credit profile.
- **Credit tiers:** Three tiers — Excellent, Good, Fair. Excellent credit typically qualifies for higher limits and longer interest-free periods. Good and Fair may qualify for lower limits or shorter promotional terms. All subject to approval.
- **Co-signers:** Co-signers are welcome and can improve approval odds or increase available credit for applicants with limited or challenged credit history.
- **Minimum requirements:** Must be at least 18 years old, no bankruptcy in the last 5 years, no collections in the last 3 years, minimum monthly income of $1,500.
- **Maximum approval:** Up to $25,000 or more with excellent credit.
- **Decline backstop:** Applicants not approved for the Advance Care Card may still qualify for a medical installment loan through mymedicalfunding.com, a preferred alternative financing partner with its own approval criteria.
- **Where accepted:** The card is accepted at enrolled healthcare providers across the United States.
- **Provider enrollment:** Free for providers to join; no setup or monthly fees to participate.
